Physician/Physician Assistant or Physical Therapy Appointments

Our direct appointment line number for physician or physician assistant appontments is (540) 372-4221.  Our appointment specialists are available between 8am and 4:20 PM Monday through Friday.

As for physical therapy appontments, you may call our appointment line at (540) 479-4764.  Our appointment specialists are available between 8am and 4:20 PM Monday through Friday.

Please have available the following information:

  • Patient's full name, date of birth, and social security number, if known.
  • Complete insurance information - this allows the Appointment Clerk to accurately inform you of any special insurance requirements such as the need for a referral and to determine if your physician participates with your insurance plan.
  • Patient/Guarantor daytime phone number/s for notification of any appointment change or cancellation.
  • Worker's Compensation patients - a phone number and name of your employer in order that we may verify your Worker's Compensation date of accident and billing information.
  • Previous X-rays, MRIs or Scans taken, which are related to the condition for which the appointment is being scheduled.  Most often, it is required for the patient to bring such films with them at the time of the appointment.

Please call well in advance to schedule your appointment.  This will better assure that an appointment can be scheduled at a time that is convenient for the patient.

Mary Washington Hospital Referred Patients:

  • Please contact our office within 24 hours after being released from ER or Hospital care.  You are required to follow all standard appointment guidelines outlined in this section.  Please make sure you obtain a valid referral.  Please do not assume that being treated through the hospital or ER eliminates or changes your referral requirements.  Any patient treated through the ER is required to obtain all X-ray films taken at the hospital, which are related to the current treating condition.  Please present these films to the receptionist at the time of the appointment.   Hospital personnel will not deliver these films for you.  Failure to obtain these films may require that the patient be rescheduled.


  • Our Appointment Clerks will do their best to notify you of any required referral.  However, it is the patient's/guarantor's responsibility to contact their insurance carrier and primary care physician to obtain referral requirements and status of any pending referrals.  The patient/guarantor is required to provide the receptionist with an original or copy of a valid referral prior to the appointment date and time, (exception: Aetna patients).  Aetna referrals are processed electronically.  All Aetna patients should contact our referral specialist by dialing our direct line (540) 372-4224 to confirm the completion of their referral or confirm that the previous referral is still valid.  Failure to produce a valid referral may require the patient's appointment to be re-scheduled.  Any patient who chooses to be treated without proof of a valid referral will be required to sign a waiver and make payment in full at the time of service.

Liability Claims (Example: Automobile Accident Claims):

  • Since we do not file third party insurance claims, the patient/guarantor will be required to file personal health insurance or personally pay in full for all charges at the time of service.  We will be glad to provide a copy of any charges and payments in order for the patient/guarantor to be reimbursed by the third party payer.  We recommend that you follow all normal processes required by your personal health insurance carrier such as acquiring proper referrals and pre-certifications.  We request that you inform our billing department of any legal representation in your behalf and what information is to be released to that entity.  Please ask your legal representative to have you sign all forms required by HIPPA regulations, which will allow us to properly release all requested information without delay.

Worker's Compensation:

  • You are required prior to your appointment to provide complete Worker’s compensation information. If this information is not received prior to your appointment, your appointment will be rescheduled.
    • Employer's name, address, and phone number.
    • A contact person at your place of employment who can confirm your date of accident and confirm that you have filed an accident report.
    • Worker's Compensation billing information. We are required to bill your employer's Worker's Compensation carrier directly.
    • Assigned claim number/s related to your current Worker's Compensation injury.
